Revised Monster Index Sample Page

I've been tweaking the page layout for the B/X Monster Reference Index. Each entry now includes a superscript 2 character reference to help identify the books the monsters come from. I've also added ascending AC values in brackets after the descending AC value.

Here's the reworked layout:

      Today's actually the first time I actually took a proper look at the ascending AC history. Even though I see the logic of it, I can't undo 35 years of thinking that AC2 for a fighter in plate is just plain right. Glad we can include both though.
